Complaints

Use and disclosure of your medical information:

Breach notification: In the event of a breach of your unsecured Protected Health Information, we will notify you as required by law.
Right to request restrictions: You have the right to request restrictions on the use or disclosure of your medical information for treatment, payment, or healthcare operations.
